Community Specialist Palliative Care Team (Macmillan)
Self referrals are accepted as well as referrals from health professionals who may be involved in the patient care.
On receipt of referral either a clinic appointment is sent to the patient or a letter introducing the team which will then be followed by a telephone call and a home visit arranged as necessary.

Helen Roberts
CLINICAL NURSE MANAGER (Macmillan)
Since November 2010 Helen has been working with the Community Specialist Palliative Care Nuses as their Clinical Nurse Manager (Macmillan). Before this, from 1997 to 2010, she was working within the Hospital Specialist Palliative Care Team in Cheltenham General as a Clinical Nurse Specialist having been a Ward Sister on one of the Oncology wards in Cheltenham General.
Helen qualified as an RGN in 1975 and went on to gain ENB certificate in oncology at the Royal Marsden Hospital in 1978 and subsequently certificates in teaching and assessing, care of the dying, quality of nursing care, ethics and law, counselling and management. More recently obtained a diploma in palliative care, BBC in community studies and advanced practice modules in independent/supplementary nurse prescribing and health and diagnostics.
