How to use the Publication Scheme
Throughout our Scheme there are references to key documents held by Gloucestershire Hospitals NHS Foundation Trust. If you are viewing this Scheme on the Internet to access the documents, simply click on the hyperlink or use the given website link. If the information you require is not accessible through a hyperlink to our website or that of another organisation, please complete the online
request form or if you do not have access to the internet, contact the Trust Freedom of Information Lead for a form.
Information can only be processed if you provide, in writing, your name
and address. Once the details have been processed, the Publication Scheme
Co-ordinator or a lead person within a service or department who has
a responsibility for responding to requests under the Freedom of Information
Act may invite you to view the information or will send you a copy*. Again
some information may be exempt from disclosure.
* We aim to provide you with the information you need within twenty working days. If this is not possible, we will tell you why. Although some of the information on the Publication Scheme may be able to be provided free of charge, for other there may be a charge. Payment must be received before the information can be retrieved. Where information is to be provided at a cost the charges will be calculated as set out in Class 15.
